A QUESTION to be put to Full Council revisits a long running battle of a bereaved mother fighting for the full background as to why her daughter died while in the care of Pembrokeshire County Council.

Sarah Pollock, from Haverfordwest, has been calling on the Local Authority to let her see the full report of a Serious Case Review carried out following the death of her 14-year-old daughter Seren Bernard in 2012.

Seren killed herself while in the care of the council despite her mother’s best efforts to bring her home.

A question submitted by Ryan Dansie states: “In 2012, 14 year old Seren Bernard committed suicide whilst in the care of PCC. The full Overview report from the serious case review has been requested by the mother yet she continues to be denied access to the report. In a statement to ITV Wales PCC said that they were following Welsh Government guidelines by not making the full report public.

“Does this new cabinet agree that the mother has a right to know what happened to her daughter whilst in the care of PCC and will this cabinet help to push for the full report to be released to the mother?”

A summary report of a serious case review into Seren’s death was published in January 2014 and found that there were a number of lessons to be drawn from the case but added it “must remain uncertain whether there were any steps that, if taken, would have prevented” her death.

Full Council will be held on Thursday, July 20, at County Hall, 10am. It will also be available to watch via the council webcast.
